Policies on the Transport of Patients

  1. . ADMITTED AT THE WARD
    1. The male nursing attendant assigned to the ward where the patient is admitted will be responsible for transporting patient.
    2. The hemodialysis machine and other pertinent equipment must be ready before the transport of patient from the ward to the unit.
    3. If the patient is in critical condition , the resident-in-charge should accompany the patient during transport.
    4. After the hemodialyis, the HDU nurse shall infrom the ward nurse about pertinent information before transporting the patient back to the ward
  2. STILL FOR ADMISSION
    1. Patient undergoing dialysis procedure and develops complication warranting admission must be endorsed to the emergency room by the Nephrology resident assigned at the unit with the latter being responsible for making the admitting notes.
    2. In cases when the patient shall be admitted , the hemodialysis sheet with nurse's notes at the back should be attached to the chart
    3. The hemodialysis technicians are responsible for transporting the hemodialysis patient to the emergency room
